The Upper Arlington Boosters Grant Request Form

Applications for funding will be accepted and reviewed by the Boosters Board. Applicants will be notified by email, text or phone call as to the decision on their grant. Completed requests should be submitted to: UA Boosters Grant Request, c/o Grants, 1625 Zollinger Road, Unpper Arlington, Ohio 43221. As of 2020, Boosters will not consider grant requests for uniforms.
